Self-review posts a GitHub check

Each self-review now reports a sinatra / review check on the PR: in progress while it runs, failing on P0/P1 findings, neutral on P2/P3, passing when nothing remains.

The check sits on the commit next to your CI, so you can glance at it or make it a required check. We ignore our own check when watching CI, so a failing review does not start a CI-fix loop.

It needs Checks write access on the GitHub App. Existing installations have to approve the updated permission; until they do, the check is skipped and the rest of the run carries on.
