Self-review runs twice, and the check can go green
After self-review applies fixes, or a follow-up adds no new issues, we run an incremental second pass and turn sinatra / review green when nothing new remains.
The first pass still posts leftover P0/P1 comments. Those stay on the PR. What
changed is the check: a response that does not introduce new issues no longer
leaves sinatra / review stuck red from the original findings.
