Linear keeps the actionable task failure after a PR opens
A failure after a pull request opens no longer gets replaced by a state-transition error in Linear.
When a sandbox or credential operation failed after a task reached review, the failure bookkeeping could raise a second error and obscure the cause that needed attention. We now leave the open pull request state intact and show the original safe failure message; internal details that are not approved for the ticket remain redacted.
