Plan documents stay in the conversation

A plan-only reply could say that a document was written without delivering the document itself. Once the agent's reply is posted, we now attach the plan or requirements file it cites.

The skills also tell the agent to publish the full document as its answer, rather than stop at a sandbox file path. File-size, credential, and path checks still apply; a document that cannot be attached gets an explicit note instead.
