An expired Codex connection now asks you to reconnect

When ChatGPT stops accepting your Codex connection's refresh token, Sinatra now shows the connection needs reconnecting instead of still calling it connected.

Before, we treated ChatGPT's usual form of that rejection as a temporary error and retried every hour, so the connection looked fine until a task failed with a generic error. Now a task that hits it fails with the same message as a missing connection, which tells you to connect Codex in workspace settings. Reconnecting there fixes it. If ChatGPT rejects Sinatra's own app rather than your account, we keep your connection and retry instead.
